Diabetic Program Information

2025 Diabetic Program Information:

Requirements for the District's 2025 diabetic wellness program (LBD) for the reimbursement component are listed below (failure to meet guidelines each month as well as specific requirements/due dates listed below will result in reimbursement(s) being denied):

  •  Adult Plan Participants - Semi-annual physician visits for adult diabetic patients relating to diabetic diagnosis with proof of visits provided to Risk Management Benefits Clerk

    • 1st semi-annual physician visit due by June 30th, 2025.

    • 2nd semi-annual physician visit due by December 31st, 2025.

    • Physician visits must include labs.

  • Adult Plan Participants - Proof of an annual foot check from a physician (must be submitted/completed to Risk Management Benefits Clerk by the end of June 2025).

  • 17 years of Age or Younger Plan Participants - Quarterly endocrinologist documented visits for adolescent or younger diabetic patients with proof of visit.

    • January-March = 1st Quarter; April-June = 2nd Quarter; July-September = 3rd Quarter; and, October-December = 4th Quarter.

  • All Plan Participants - Monthly education completed within the month assigned, i.e. January education completed in January.

    • This includes parent/child sign off of education provided/overseen by the parent to the child regarding the wellness education/program that month through iaWellness (for dependents 17 and under, they will participate via the employee’s wellness account).

  • All Plan Participants - Monthly education program (through iaWellness) will also require a monthly attestation of physical exercise/activities completion by the participant (at least 10 times during the month) beginning in January 2025 and thereafter.

  • All Plan Participants - Proof of an annual dental exam provided to Risk Management Benefits Clerk (must be submitted/completed by the end of June 2025).

  • All Plan Participants - Proof of annual dilated eye exam and/or diabetic eye exam specific (must be submitted/completed to Risk Management Benefits Clerk by the end of June 2025).

    • If your physician does not recommend a required exam for you, please submit signed documentation from your physician stating such.

  • All Plan Participants - Information sheet completed and provided to Risk Management Benefits Clerk (who will provide a copy to iaWellness) before January 30, 2025. The information sheet and program outline has been sent out to current participants the first week of December and posted to the webpage in December 2024.

    • If new to the Lubbock ISD Health plan or newly diagnosed diabetic, (and prior to any processing of a diabetes related expense) the completed information sheet will need to be provided within 30 days of enrolling in the Lubbock ISD Health plan or 30 days from diagnosis.

  • All Plan Participants - Annual dietician/nutritionist education completion with proof of education provided to Risk Management Benefits Clerk (must be submitted/completed by the end of June 2025).  Please note that iaWellness does have a nutritionist available for consultation(s) for participants over the age of 17.

The purpose of the LBD program is ongoing and consistent care of oneself and not solely to submit a large claim for reimbursement once or twice a year – consistent monthly participation in the program is required each month as well as specific requirements and due dates listed above in order to receive reimbursement(s) – the requirements will be enforced and followed by the District and iaWellness.

2025 Living Better with Diabetes Claim Form (Employee/Spouse/Dependent Health Plan Members Only)

See the requirements above and watch the required monthly videos and complete the monthly physical attestations through iaWellness.

Employee, spouse, and dependent health plan members (ONLY) who participate monthly in the Living Better with Diabetes Program can receive up to $2,500 annual reimbursement for diabetes related costs. Up to $100 for eligible physician visits.

Steps to take:

  • First, contact Risk Management with any questions and be sure you complete the Information Sheet/2025 Program outline provided on this webpage and send it to Risk Management Benefits Clerk asap (before January 30, 2025, or within 30 days of joining the Lubbock ISD Health plan or 30 days from date of diabetic diagnosis).

  • Reach out to iaWellness at 806-765-7265 or info@iawellness.com to ask about Living Better with Diabetes Program participation. (Additional monthly participation required to be eligible for reimbursements.)

  • Download the 2025 claim form to print and complete (all areas must be completed by the employee/claimant or the claim will be denied and sent back).

  • Attach backup information to your claim form showing services you'd like to claim. (Examples of backup: a pharmacy slip showing the date, drug information and cost, or a paid receipt from a medical provider's office with the accompanying explanation of benefits.)

  • Submit the completed claim form and backup information (receipts, etc.) to Risk Management Benefits Clerk. Fax 806-766-1195 (scanned emails must be PDFs and not images.)

  • Be sure you submit all other required documents such as proof of annual dental visit, diabetic eye visits, dietician visit, etc. according to the timeline above and on the information sheet/agreement signed by the participant.  Failure to meet the requirements and due dates will result in denial of the claim(s).

HIGHLIGHTS ABOUT CLAIMS PROCESSING:

***Diabetes claims must be submitted with all required documentation supporting the claim (i.e., pharmacy slips, receipts, etc.) along with the completed LBD form within sixty (60) days from the date of payment for processing.  Claims submitted after sixty (60) days from the date of payment will be denied.***

  • The Monthly Living Better with Diabetes Education programs are available on the wellness portal each month

  • Register your account (if you have not already)

  • Scroll down to the section "Living Better with Diabetes Monthly Program"; hover over the month's education:

    • Click "Learn"

    • Click "Take Course"

    • View the video, and scroll down to answer the short test questions

    • Once the monthly video is complete, go back and complete the monthly physical attestation.

  • Please remember, the courses will only be available in the "Living Better with Diabetes Monthly Program" section in the month it was assigned

  • You will receive an email upon completion of the course

  • Please forward the completion emails from the courses to Risk Management Benefits Clerk to facilitate the Living Better with Diabetes claims process

  • Effective 4/1/2023 and ongoing, all monthly courses MUST BE completed in the month the course is released in order to gain credit for the Living Better Diabetes claims processing or the claim will be rejected for non-participation.  Again, failure to complete the assigned class in the month assigned will result in rejection of your claim(s) for reimbursement for that month - no exceptions - you are no longer able to go back and make up classes for reimbursement for a previous month's education program.  The goal of the Living Better with Diabetes program is for participants to manage their diabetes plan, education, etc. on a concurrent basis

  • All Living Better with Diabetes claims must be submitted within 60 days from the date of payment to be eligible for reimbursement - claims outside the 60 days from the date of payment time frame will be rejected

  • Beginning in 2024, employees, spouses, and dependents covered on one of Lubbock ISD's medical plans are the only plan participants eligible to participate in the Living Better with Diabetes Program and Living Better Diabetes reimbursement process

If you have not registered your account and need a new registration link or have trouble accessing your account or the monthly LBD education, please contact iaWellness or call 806-765-7265 for assistance.  If you are a spouse or dependent over the age of 17 and participating in LBD, ask the employee health plan member to send you an invitation to join!  They can go to the top right corner to access their account information and click "Family" to invite you (for dependents 17 and under they will participate via the employee’s wellness account)
