Beginning of the Year Paperwork

Below is a list and description of the paperwork and forms you will find in your student's beginning of the year folder:

Your student will receive a different liability release waiver for every internship in which we participate throughout the school year.  Although we will not begin our vocational internships and community outings until after Labor Day, it is important to have these liability release waivers signed at the beginning of the year so that we can begin our internships as soon as our community partners are ready to welcome us.

  • Lubbock ISD Over-The-Counter Non-Prescription Medication Release Form

This form lists various over-the-counter (OTC) medications and allows parents to designate which OTC medications the VTC school nurse is allowed to administer as needed to the student during the school day.  The student's parent or guardian must sign this form and indication which medications are allowed in order for the VTC school nurse to be able to administer OTC medication to the student at school.  

Request for Food Allergy Information

This form is very important as our students participate in bi-weekly cooking labs and we want to make sure that we prepare foods that are accessible to all students.

Parent Communication Form

This form allows parents to indicate their desired method of contact for events such as school events, changes to the bus schedule, or a student illness resulting in a visit to the school nurse.

Agency Contact Information

This form requests that parents list all outside agencies which may be providing additional services to your student such as Texas Workforce Commission, StarCare, Class Program, etc.  This information helps us when preparing for your student's IEP meeting.

Media Release Waiver

This form allows for VTC staff members to take pictures or videos of your student.  These pictures are only used within the student's instructional program and are not distributed outside to any agencies or personnel outside of Lubbock ISD.

VTC T-shirt order form

Although students are not required to purchase the VTC t-shirt, it is encouraged.  Students will need to wear their VTC t-shirt for large group field trips and for Special Olympic events.
