• $37.00 per week, Full Time (6 or more hours per day)

• $24.05 per week, Part Time (less than 6 hours per day)

• Shot Records

• Doctor and Hospital information (name, address, phone number, etc.)

• Name, address, and phone number of Person Authorized to pick-up child

• Proof of income

• TANIF and/or Food Stam Case Numbers
